Memorial Brick Order Form

Those who donate $300 or more will have the opportunity to add a personal message to one of the commemorative bricks forming the foundation of the statue.

Each brick is laser printed for an enduring memorial. Price includes brick placement at the St. Louis Firefighter Memorial located at St. Louis Fire Department Headquarters.

The St. Louis Firefighter Memorial Foundation is a 501(c)3 and all donations are tax-deductible. If you would like a receipt or you are part of a corporation interested in making a donation directly, please email StLFDMemorialFoundation@gmail.com.

You can choose from two options:

  • $300 - 4” x 8” Brick
    You may only use up to three lines of type (20 characters including spaces)

  • $500 - 8” x 8” Paver
    You may use up to six lines of type (20 characters including spaces)

You may order a brick by using the form below or by filling out a physical form and mailing it.

Once you complete the form, please submit payment electronically via Cashapp ($STLFFM) or Square. You may also send check, money order, or cashier’s check (made payable to St. Louis Firefighter Memorial Foundation) to St. Louis Firefighter Memorial Foundation, PO Box 39353, St. Louis, MO 63139. With all payments, please include the name and phone number used in your submission.

The SLFMF reserves the right to refuse the purchase of bricks by any individual or organization as well as approve the content to be placed on the bricks.
